SterRx, LLC Issues Voluntary Nationwide Recall of Certain SterRx Products
SterRx, LLC today announced the voluntary nationwide recall of approximately 240 lots within their expiry period due to equipment and process issues that could lead to a lack of sterility assurance for products intended to be sterile. To date, SterRx, LLC has not received reports of any product complaints or adverse events associated with this issue. SterRx, LLC has initiated this voluntary recall to the hospital pharmacy level out of an abundance of caution.
This recall is being conducted with the knowledge of the U.S. Food and Drug Administration.
The lot numbers being recalled were distributed to hospitals nationwide from December 2020- October 2021. The products impacted are specifically:
lmg/ml Midazolam in 0.9% Sodium Chloride
Fentanyl in 0.9% Sodium Chloride
lmg/ml Morphine Sulfate in 0.9% Sodium Chloride
lmg/ml Morphine Sulfate in 5% Dextrose
125mg Diltiazem HCL in 0.7% Sodium Chloride
125mg Diltiazem HCL in 5% Dextrose
Norepinephrine in 0.9% Sodium Chloride
Norepinephrine in 5% Dextrose
Epinephrine in 0.9% Sodium Chloride
Phenylephrine in 0.9% Sodium Chloride
150mEq Sodium Bicarbonate in 5% Dextrose
200mg Succinylcholine Chloride in 0.9% Sodium Chloride

Alsum Farms & Produce Voluntarily Recalls Select Yellow, White, and Red Onions Because of Possible Health Risk
Friesland, Wisconsin, November 12, 2021 — Alsum Farms & Produce, Inc. of Friesland, Wisconsin is voluntarily recalling Alsum Farms & Produce fresh whole yellow onions in 3 lb. bags, 5 lb. bags, and 50 lb. cartons; fresh whole white onions in 2 lb. bags; and fresh whole red onions in 2 lb. bags delivered to select retailers in Wisconsin, Illinois, Michigan, and Pennsylvania between July 13, 2021 and August 18, 2021. The recalled onions were imported from Chihuahua, Mexico. This only affects onions labeled “Produce of Mexico”. The recall does not impact any other Alsum Farms & Produce products.
“The health and safety of our customers is our top priority," said Heidi Randall, COO of Alsum Farms & Produce. “We advise consumers check their home pantries for any recalled product remaining to minimize even the slightest risk to public health.”
The affected onions were sourced from Keeler Family Farms and are being recalled because they have the potential to be contaminated with Salmonella, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (i.e., infected aneurysms), endocarditis and arthritis. Thoroughly cooking fresh onions to 165°F/ 74°C will kill the Salmonella bacteria.
Potandon Produce Updates Voluntary Recall of Select White and Yellow Whole Onions Because of Possible Health Risk
Potandon Produce L.L.C. of Idaho Falls, Idaho is updating its October 26 voluntary recall of onions supplied from Keeler Family Farms and sold as Green Giant Fresh to include 3lb. and 5lb. bags of whole yellow onions and 2 lb. bags of whole white onions shipped between July 15 and August 22, 2021 to a UNFI retail distribution center in Champaign, Illinois. This recall does not affect any other Green Giant Fresh products or include any Green Giant canned or frozen vegetable products.

Oregon Lox Company Recalls Wild Cold Smoked Keta Salmon Lox Because of Possible Health Risk
November 11, 2021, Oregon Lox Company of Eugene, Oregon is issuing a voluntary recall of various brands of Wild Cold Smoked Keta Salmon Lox, specific lot number 22821, because it has the potential to be contaminated with Listeria rnonocytogenes, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Although healthy individuals may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea , abdominal pain and diarrhea, listeria infection can cause miscarriages and stillbirths among pregnant women.
The affected Wild Cold Smoked Keta Salmon product was distributed in Alaska, California, Oregon, and Washington through dis tributors. Product was shipped frozen on common carriers.
ILG International Trading Inc. Issues Alert on Undeclared Sulfites in “Natural Delight Collection” Natural Dried Apricots
ILG International Trading, Inc of Brooklyn, NY is recalling its 500 g (17 oz) packages of “Natural Delight Collection” Natural Dried Apricots because they contain undeclared sulfites. Consumers who have severe sensitivity to sulfites run the risk of serious or life-threatening allergic reactions if they consume this product.
The recalled “Natural Delight Collection” Natural Dried Apricots, product of Uzbekistan, were distributed nationwide in retail stores. The product comes in a 500 g (17 oz) cardboard box marked with a best before date of 03/30/2022. The product UPC code is 811579000533.
Nutracap Holdings; LLC Issues Voluntary Recall of Various Dietary Supplements Due to Undeclared Milk
November 12, 2021 – Nutracap Holdings, LLC of Norcross, Georgia is voluntarily recalling various dietary supplements due to undeclared milk on the labels. People who have an allergy or severe sensitivity to milk run the risk of serious or life-threatening allergic reaction if they consume products containing milk.
During a recent FDA inspection, the firm was notified that their labels failed to disclose the presence of milk on some of their products.
The recalled products were shipped between January 2020 through November 2021 to distributors and retailers in Arizona, California, Colorado, Florida, Georgia, Idaho, Illinois, Indiana, Kentucky, Louisiana, Minnesota, Nevada, New Jersey, New York, North Carolina, Ohio, and Texas.
Ellenos Real Greek Issues Allergy Alert on Undeclared Egg in Mango Greek Yogurt 16 oz. Cup
November 10th, 2021, Ellenos Real Greek Yogurt of Federal Way, WA is recalling Mango Greek Yogurt 16 oz. cups because it may contain undeclared egg. People who have an allergy or severe sensitivity to egg run the risk of serious or life-threatening allergic reaction if they consume this product.
Mango Greek Yogurt 16 oz. cups were distributed in Oregon and Washington through select retail stores.
The Mango Greek Yogurt 16 oz cup has the UPC 8 57290 00612 6 and has the affected BEST BEFORE date of 11/28/2021 printed on a blue foil seal. This is a refrigerated item and packaged in a clear plastic container with a blue foil seal and can be identified by the blue lettering on front panel display. No other Ellenos brand yogurt products of any other size or similarity were affected.

November 9, 2021 – Nutracap Holdings, LLC of Norcross, Georgia is voluntarily recalling various dietary supplements due to undeclared wheat, milk, soy and/or coconut on the labels. People who have an allergy or severe sensitivity to soy, milk, wheat, or coconut run the risk of serious or life-threatening allergic reaction if they consume products containing soy, milk, wheat, or coconut.
On 11/4/2021 during an FDA inspection the firm was notified that their labels failed to disclose the presence of allergens on some of their products.
The recalled products were shipped between June 4, 2020 through October 1, 2021 to distributors and retailers in Alabama, California, Florida, and Texas.
