Merck Issues Voluntary Nationwide Recall of CUBICIN® (Daptomycin for Injection) 500 mg, Lot 934778, Due to Presence of Particulate Matter Identified as Glass Particles
KENILWORTH, N.J., Oct. 19, 2021 – Merck (NYSE: MRK), known as MSD outside the United States and Canada, is voluntarily recalling one lot of CUBICIN® (daptomycin for injection) 500mg for intravenous use, Lot 934778, Exp June 2022. Because treatment with CUBICIN is likely to be initiated at a hospital or other monitored healthcare setting, the recall is being conducted to the user level, including hospital and administering institutions. Approximately 22,000 vials are affected by this recall. The recall has been initiated following receipt of a customer complaint reporting that a piece of glass was found in a vial of CUBICIN after reconstitution.
Intravenous infusion of glass particulates has the potential to cause serious health consequences if the particulate is small enough to be withdrawn from the vial and infused into the patient. Local irritation or swelling at the infusion site may occur in response to the presence of foreign material. More serious potential outcomes include blockage and clotting in blood vessels, which can be life-threatening if a critical organ is affected. Other clinical consequences could include prolonged hospitalization, particularly in those patients receiving an extended treatment regimen for which multiple vials of CUBICIN are administered over the course of treatment. The risk is reduced by the possibility of detection, as the label contains a clear statement directing the healthcare professional to visually inspect the product for particulate matter prior to administration. To date, Merck has not received any reports of adverse events related to this recall.
Bryant Ranch Prepack Issues Voluntary Nationwide Recall of Methocarbamol 500mg Bottles Due to Mislabeling With the Incorrect Strength
Burbank, CA, Bryant Ranch Prepack is voluntarily recalling 1 lot of Methocarbamol 500mg, Tablets to the consumer level. The bottles labeled as Methocarbamol 500mg Tablets have been found to contain Methocarbamol 750mg Tablets.
Risk Statement: If a patient takes a 750mg Tablet of Methocarbamol instead of the prescribed 500mg Tablets, it potentially could result in Excessive Central Nervous System depression which may result in nausea, sedation, fainting, falls, seizure, coma, and death. Bryant Ranch Prepack has not received any reports of adverse events related to this recall.
The product is used together with rest, physical therapy, and other measures for the relief of discomfort associated with acute, painful musculoskeletal conditions and is packaged in a white round bottle with a red and white label, which reads Methocarbamol 500mg packaged in counts of 30 (NDC:7133517952), 60 (NDC: 7133517954), and 90 (NDC:7133517957) pills. The affected Methocarbamol 500mg lots include the following Lot Number 163935/ Exp. Date 10/22. The product can be identified by red and white label with a yellow border at the top and bottom of the label, top of the label reads “Packaged by Bryant Ranch Prepack”, labels are pictured below. The Methocarbamol 500mg was distributed Nationwide to multiple physician offices.

Potandon Produce Voluntarily Recalls Select Red, White, and Yellow Whole Onions Because of Possible Health Risk
Potandon Produce L.L.C. of Idaho Falls, Idaho is voluntarily recalling Green Giant Fresh whole yellow onions in 2 lb. bags, 3 lb. bags, and 5 lb. bags; whole white onions in 2 lb. bags; and whole red onions in 2 lb. bags delivered directly to three UNFI retail distribution centers (DCs) in Fargo, North Dakota; Bismarck, North Dakota and Hopkins, Minnesota between July 9 and Aug 6, 2021. This recall does not affect any other Green Giant Fresh products or include any Green Giant canned or frozen vegetable products.
The recalled onions were sourced from ProSource Produce LLC and are being recalled because they have the potential to be contaminated with Salmonella, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (i.e., infected aneurysms), endocarditis and arthritis.

Smilin’ Bob’s Issues Allergy Alert On Undeclared Egg In Smilin’ Bob’s Smoked Fish Dip Products
Smilin’ Bob’s is initiating a voluntary recall because a limited amount of Smilin’ Bob’s Original Smoked Fish Dip was mistakenly packed in Smilin’ Bob’s Natural Smoked Fish Dip cups with a Original Smoked Fish Dip lid. We discovered the issue when one of our retail customers brought to our attention that cups had a UPC code that did not match that for the “original” fish dip. As a result, the packaging does not list the presence of a possible egg allergen. Some people who have an extreme allergy or severe sensitivity to egg could run the risk of a serious or life threatening allergic reaction if they consume this product.

October 20, 2021, ProSource Produce LLC of Hailey, Idaho, is voluntarily recalling whole raw onions (red, yellow, and white) shipped from Chihuahua, Mexico, between July 1, 2021, and August 31, 2021, because they may have the potential to be contaminated with Salmonella, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. The products are whole raw onions (red, yellow, and white) that were shipped to the United States from Chihuahua, Mexico, between July 1, 2021, and August 31, 2021, and were distributed in AL, AR, AZ, CA, CO, CT, FL, GA, IA, ID, IL, IN, KS, KY, LA, MA, MD, MI, MN, MO, MS, NC, NE, NJ, NM, NY, OH, OK, PA, RI, SC, TN, TX, VA, and WI, as well as Ontario and Quebec, Canada. No fresh onions that originated in Chihuahua, MX have been shipped by the company since the end of August 2021.

The Ivar’s Kettle Classic Clam Chowder soup is packaged in 24 oz clear plastic cups, in a double pack surrounded by a printed paperboard sleeve and has a UPC 0 30383 19649 6. The ready-to-eat soups have a Use By Date of 12-22-2021 printed on the bottom of the sleeve, and on the edge of the individual cup lids.

Cook Medical Issues Voluntary Recall of Transseptal Needle and Transseptal Needle with Catheter
Bloomington, Ind. — On October 8, 2021, Cook Medical issued a global, voluntary recall of the Transseptal Needle and the Transseptal Needle with Catheter. This recall includes all unexpired lots for both of these products. The needles were recalled due to complaints of rust on the products. Use of affected products could result in increased procedural time and inflammatory reactions, including systemic reactions which may lead to permanent impairment or death.
